サイト共通の登録入口です。今回は項目設計と説明だけを先に配置しています。

This is the shared sign-up entry point. For now it focuses on the form structure and guidance only.

送信処理はまだ入れていません。Firebase Authentication もしくは既存API接続を決めた段階で有効化します。
Submission is not enabled yet. The form can be activated later once Firebase Authentication or an existing API is selected.
